Removable Windshield Placard
Eligible veterans, and active-duty members of the Armed Forces of the United States, may qualify for a gratis (free) removable windshield placard. These removable windshield placards permit vehicles to park in parking spaces marked with the international symbol of access for persons with disabilities.
Eligibility
- Active-duty service member or veteran with a documented service-connected disability that limits or impairs the ability to walk.
Restrictions
- A second placard request must state the reason why an additional placard is necessary.
Documents Needed Based on Military Status
- BMV 4531 Application for Removable Windshield Placard for Active-Duty Military / Veterans With Disabilities
- Letter, dated within one (1) year, from the U.S. Department of Veterans Affairs, indicating that the applicant’s disability is service connected.
- One of the following 2 options:
- Prescription from a health care provider. The prescription must contain:
- Date
- Name of the person with the disability
- Statement that the prescription is for a removable windshield placard
- Expected duration of the disabling condition
- Health care provider’s signature
- Section C of the BMV 4531 completed by a health care provider.
- BMV 4531 Application for Removable Windshield Placard for Active-Duty Military / Veterans With Disabilities
- Department of Defense convalescent leave statement or other documentary evidence supporting illness, injured casualty status, or placed on limited duties.
- One of the following:
- Prescription from a health care provider. The prescription must contain:
- Date
- Name of the person with the disability
- Statement that the prescription is for a removable windshield placard
- Expected duration of the disabling condition
- Health care provider’s signature
- Section C of the BMV 4531 completed by a health care provider.
Process
In-Person
- Visit the nearest BMV deputy registrar license agency with the required documents to complete the transaction.
By Mail
- Send all necessary documents to Registration Support Services at the address identified on the Application.
- Allow 10-15 business days for processing and to receive the placard in the mail
