Applicants for an Ohio driver license or identification (ID) card will be asked if they have executed a valid power of attorney for health care and a declaration for the use of life-sustaining equipment. They will also be asked if they wish the driver license or ID card to indicate that a power of attorney and life-sustaining equipment declaration have been executed.
