What documentation do you need to have an Ohio ID card issued for your child?

The applicant (child) must provide proof of the following:
• Full legal name
• Date of birth
• Social Security number (if one has ever been assigned)
• Legal presence (U.S. Birth Certificate, valid U.S. Passport/Passport Card, Naturalization Papers, USCIS documents)
• Residency

Any child under the age of 18 years old must be accompanied by a parent or guardian, who must also provide proof of name, date of birth, and Social Security number, if ever assigned, or their Ohio driver license. http://bmv.ohio.gov/idrkids.stm

Pattie Stechschulte's plate reads 4MYJCK to honor her 16-year-old son's brave struggles with autism. Pattie says Jack's autism never gets him down and she thinks her plate is a great way to celebrate him.
