Any owner or lessor who is a
resident of this state, and holds an un-revoked and unexpired medical
license admitting them to practice medicine, upon application may purchase
these plates. |

Registrant Eligibility:

A copy of a current valid Ohio
license to practice medicine in Ohio. |

Title Requirements: |
Ohio Certificate of Title issued
in individual's name, either singly or jointly, or the name is the same name
on the lease agreement. |

Vehicle Information: |
These plates may be issued to any
motor vehicle. |

Type of Plates: |
System assigned plate number,
initial reserve and personalized. |

Issuance Information: |
Requests for system assigned,
reserved, or personalized
Physician
license plates may be made at any
Deputy Registrar's office.
Renewal of plates can be
done at a
Deputy Registrar office,
online at
OPlates.com,
by phone
1-800-589-TAGS
(1-800-589-8247) or
by mail.
All plates will be mailed directly to the address on your registration card
unless customer requests otherwise. The deputy will issue your registration
card, stickers and provide you with a permit which will allow you to operate
your vehicle while your plates are being manufactured.

license agency locations or the Request for Special Plates application

Cost: |
There is a one-time fee for
physician plates of $10.00, in addition to normal registration fees. The fee
is determined by Ohio Revised Code section 4503.15. |
