Any private emergency medical
service organization as defined in section 4766.01 of the Ohio Revised Code,
may apply for these plates. |

Registrant Eligibility:

A copy of the
vehicle acquisition form or vehicle listing sheet - with the Red Stamp
affixed (A vehicle must be re-inspected every years).

A
copy of the current wall certificate (certificate of licensure) issued by
the Ohio Medical Transportation Board with the Red Stamp affixed.

Title Requirements: |
Ohio Certificate of Title issued
in company's name, either singly or jointly, or the name is the same name on
the lease agreement. |

Vehicle Information: |
Currently, all emergency
vehicles, regardless of how titled, are registered as passenger vehicles. |
|
Type of Plates: |
System assigned plates only
inscribed with the letters: EMS Vehicle. |

Issuance Information: |
Plates must be obtained through
the Bureau of Motor Vehicles in Columbus. These plates are also issued at
deputy registrar license agencies. |

Cost: |
There is an annual fee for EMS
plates of $7.50, in addition to normal registration fees. The fee is
determined by
Ohio Revised Code section
4503.49. |
