Emergency Medical Service License Plates

EMS Vehicle Plate Image

General Information:

Any private emergency medical service organization as defined in section 4766.01 of the Ohio Revised Code, may apply for these plates.

Registrant Eligibility:

  1. A copy of the vehicle acquisition form or vehicle listing sheet - with the Red Stamp affixed (A vehicle must be re-inspected every years).

  2. A copy of the current wall certificate (certificate of licensure) issued by the Ohio Medical Transportation Board with the Red Stamp affixed.

Title Requirements:

Ohio Certificate of Title issued in company's name, either singly or jointly, or the name is the same name on the lease agreement.

Vehicle Information:

Currently, all emergency vehicles, regardless of how titled, are registered as passenger vehicles.

Type of Plates:

System assigned plates only inscribed with the letters: EMS Vehicle.

Issuance Information:

Plates must be obtained through the Bureau of Motor Vehicles in Columbus. These plates are also issued at deputy registrar license agencies.

Cost:

There is an annual fee for EMS plates of $7.50, in addition to normal registration fees. The fee is determined by Ohio Revised Code section 4503.49.
