A Teen's Guide To Getting A Driver License - GHS 0172 (Rev. 4/07) (164 KB) Informational brochure addressing safety threats for young and inexperienced drivers as well as information regarding Temporary Permits, Probationary License, Full License Eligibility, and Suspensions and Restrictions.

Acceptable Documents List - BMV 2424 (Rev. 1/08) (26 KB)  In order to obtain any Temporary Permit, Driver License, Commercial Driver License, State of Ohio Identification (ID) Card or duplicate of any of these items, the customer and any co-signer for a minor applicant must present a primary and a secondary document satisfactory to prove: (1) both name and date of birth and (2) Social Security Number (SSN), if ever assigned.

Affidavit for Designation of Beneficiary or Beneficiaries by the Sole Owner for a Motor Vehicle, Watercraft, or Outboard Motor Certificate of Title - BMV 3811 (Rev. 7/02) (27 KB) This form is completed by individuals who upon death, wish to transfer ownership by designating a beneficiary or beneficiaries of a titled vehicle, watercraft, or outboard motor.  This includes ATV's, off-road vehicles, manufactured homes, motor homes, recreational vehicles, and trailers and semi-trailers weighing in excess of 4,000 pounds.

Affidavit for "Gold Star Family" License Plates - BMV 4635 (Rev. 4/08) (31 KB) This affidavit is required to be completed by any member of the immediate family of a person who died in a combat zone while a member of any branch of the armed forces of the United States who is making application for Gold Star Family license plates.

Affidavit for Registration of Livery Vehicles - BMV 4632 (Rev. 8/04) (52 KB) This affidavit is required for all livery issuances. Any customer wanting to register a livery vehicle must submit the notarized BMV 4632 along with the appropriate proof of insurance. Customers may register livery vehicles at any deputy registrar or by calling 1-800-589-TAGS.

Affidavit for Registration - BMV 5712 (Rev. 10/07) (107 KB) This affidavit is completed by individuals registering a motor vehicle for non-commercial purposes.

Affidavit to Exempt Motor Vehicles of Volunteer Rescue Services from Registration Fees - BMV 4523 (Rev. 9/04) (98 KB) This form provides for the issuance of gratis permanent license plates for vehicles titled in the name of a volunteer rescue service certified by the Ohio Department of Public Safety and used solely in the transaction of the business of the rescue service.

Alcohol/Drug Reinstatement Form - BMV 2326 (Rev. 4/07) (39 KB) This form is only necessary for persons who have habitual/alcoholic suspensions (convicted three or more times within three years of operating a vehicle intoxicated under the influence of alcohol or drugs).

Certificate of Compliance - BMV 4347 (Rev. 10/02) (150 KB) No two motor vehicle dealers shall engage in business at the same location, unless they agree to be jointly, severally, and personally liable for any liability arising from their engaging in business at the same location. The agreement shall be filed with the motor vehicle dealers board, and shall also be made a part of the articles of incorporation of each such dealer filed with the secretary of state.

Clerk of Courts Surviving Spouse Affidavit - BMV 3773 (Rev. 3/05) (56 KB) This form is used to transfer the ownership of vehicle(s) owned by a deceased family member.

Crash Report - BMV 3303 (Rev. 1/03) (203 KB) This report may be used and sent to the BMV if a driver was in an accident and was injured or the damage was caused by an uninsured motorist. Injured parties have up to six months to send the report to the BMV.

Customer Request to Cancel Vehicle Registration - BMV 4801 (Rev. 6/07) (91 KB) This form is used to cancel a vehicle registration under certain circumstances (cancellation only - no refunds and/or credits).

Application for Assignment for "E" Dealers - BMV 3772 (Rev. 12/07) (64 KB) Permits the owner of a motor vehicle for which no physical title has been issued (E-title) to transfer ownership to a licensed motor vehicle dealer without obtaining a physical certificate of title first.  Also, this form includes an optional provision for the owner to appoint power of attorney.
