Ohio Revised Code Section 4507.081

Medical
forms will be mailed from the BMV approximately 45-60 days prior to the
expiration of the license.

Upon receipt of the completed medical form at the BMV, a
letter of instruction and a new medical restriction card, if applicable, will
be mailed to the applicant.

The Deputy Registrar will issue the license and give it to
the applicant over the counter.
If an applicant attempts to renew a medically restricted license without
a letter of instruction the Deputy Registrar will:
If an applicant indicates for the first time, at renewal, the existence
of a medical condition, the following process will be followed:
Upon receipt of the completed physician form, the BMV will
evaluate as to which type of restriction, if any, is required and mail the
proper letter of instruction and the medical restriction card, if required, to
the applicant.

If a restriction should be removed from a license, the BMV
will mail a card indicating that no restriction is required. The driver can
carry the card or return to the Deputy Registrar to have a new license made
without the restriction. If he chooses to get a new license he will be
required to pay the Deputy Registrar fee of $3.25.

In order to obtain any Ohio driver's license, commercial driver's license (CDL),
identification (ID) card or duplicate of any of these items, the
customer, and any co-signer for a minor applicant must present a
primary and a
secondary
document satisfactory to prove both: (1) Name and Date of Birth and (2)
Social Security Number
(SSN), if ever assigned. (Ohio Administrative Code Sections 4501:1-1-19, 21,
22). Additionally, non-U.S. citizens are required to provide
proof of legal
presence before an Ohio driver's license or identification card can
be issued. |

NOTICE:

Effective November 19, 2007,
individuals who are renewing a current Ohio driver license, Ohio commercial
driver license (CDL), or Ohio identification (ID) card will no longer be
required to present a Social Security card at an Ohio Bureau of Motor Vehicles
(BMV) license agency for verification at the time of renewal.
An Ohio driver license, Ohio CDL,
or Ohio ID card, which is current, or has been expired for less than six months,
is acceptable without further proof of the applicant’s Social Security number if
it has been previously verified in the BMV’s records.

Only original documents or a copy
bearing an original certification by the issuing authority are acceptable.
Uncertified copies or copies of certified documents are not acceptable. Failure
to provide two (2) acceptable documents to prove both the name and date of birth
and the
Social Security Number,
if ever assigned, shall result in denial of the application. See
