4501:1-1-04
If physician answers "YES" to allow driving privileges or question #6 on form BMV 2310, the procedures listed below will
be followed.
Examiner
enters the restriction code V in the 2-part box on the exam form, validates the
temporary permit and returns it to the applicant.
After
the road test, he instructs the applicant to proceed to the
Deputy Registrar for the
issuance of the license (with a code V) and any other common restrictions that may be
necessary.
If
applicant is the holder of a valid out-of-state license and does not have to take the road
test, he is instructed to proceed to the
Deputy Registrar for the issuance of the license
(with a code V) and other common restrictions.
Examiner
advises the applicant that the medical form will be mailed to the BMV and he will be
notified by mail of any further requirements.
Examiner
mails medical form and copy of exam or BMV 2003 flag to BMV in 2341 envelope.
After a
thorough review by BMV staff, the applicant may be:
Issued a regular license (no medical form is needed).
Issued
a restricted license requiring submission of periodic medical statements, driver
license exams; or any combination thereof.
If
physician answers "NO" driving privileges, or refuses to answer question #6 on form BMV 2310, on physician's statement, the
procedures listed below will be followed:
Applicant
advised application for Ohio driving privileges is denied.
Examiner
confiscates BMV 2015 (if it has not expired).
Examiner
mails medical forms, exam form and out-of-state license to BMV in 2341 envelope.
Applicant advised to contact BMV for another medical form when doctor is able
to approve driving.
